If you hear the term “MDS” or meet our MDS Coordinator, you might wonder what those three letters stand for.
MDS stands for Minimum Data Set. In simple terms, it is a comprehensive health assessment that we use to ensure every resident receives the highest quality of care tailored specifically to them.
How the MDS Process Works for Your Loved One:
- The Full Picture: Our MDS Coordinator gathers details from nurses, therapists, dietitians, and activity staff. This “big picture” view helps us understand your loved one’s physical health, memory, mood, and daily habits.
- The Care Plan Roadmap: The information gathered in the MDS is used to create a personalized Care Plan. This roadmap tells our staff exactly how to support your loved one—from their favorite breakfast to their specific physical therapy goals.
- Regular Check-ups: We perform these assessments upon admission, every quarter, and whenever there is a significant change in health. This ensures our care “grows” and changes right along with the resident.
- Quality Standards: The MDS is a federal requirement that helps our facility stay accountable and maintain the high clinical standards you expect.
